In association with various stakeholders in South Australia and Australia we have developed programs/trials to help implement research into practice to help promote safe, efficient and effective healthcare.

CNAHS-centric Outcomes Calculator Project

In early 2010, the Central Northern Adelaide Health Service (CNAHS) generously provided support to the International Centre for Allied Health evidence to develop and implement a standard outcome measures approach across the CNAHS region with the aim to improve the quality and safety of care of older clients. The primary target of this initiative is the transition care program (TCP) and its providers. As part of this initiative, CNAHS-centric outcomes calculator software was developed. As means of improving awareness and knowledge of outcome measures, a tailored approach involving a video-training package has also been developed
.

 

In July 2010, a workshop with selected invitees representing the Adelaide Health Service (created by the consolidation of the CNAHS and Southern Adelaide Health Service) was conducted. The aim of this workshop was to plan the operationalising of the CNHAS OC project. A key outcome of this workshop was the implementation of a pilot project at three different sites in Adelaide where the CNAHS OC will be initially trialed.

 

The structures and processes underpinning the CNAHS OC project, in an organisational context, will be trialed for a three month period (September – November 2010) at Helping Hand Aged Care, ACH Group and Southern Cross Care. Learnings from the pilot trial will then influence the wider roll out of the CNAHS OC across the Adelaide Health Service in 2011.
